Mission Statement
To provide a high quality, comprehensive palliative program dedicated to promoting comfort and quality of life for clients and families facing a terminal illness. In recognition that facing a life limiting disease is much more than a physical experience, A*MED Community Hospice utilizes an interdisciplinary team approach to provide services for their clients and families. As human beings, we experience emotional and spiritual losses as well as physical during the dying process. Through meeting the numerous needs of the clients and families, we strive to enable the client to live life to the fullest extent possible and the family to adjust to life after their loss.

A client is ready for hospice when they:

  • Have a diagnosis of a life-limiting illness.
  • Certified by a physician as having a life expectancy of six months or less.
  • Agree to no further aggressive curative treatment.
  • Agree to palliative care and comfort support.

Pain Management
A*MED Community Hospice strives to ensure our clients are as pain free as possible. Clients are individually assessed to ensure physical, psychosocial and spiritual comfort.

Hospice provides supportive care through:

Nurses -

  • Home visits to assess client's condition
  • Provide pain management
  • Available 24 hours a day

Medical Social Workers -

  • Counseling for client and family
  • Liason between patient and community agencies
  • Provide assistance with legal and financial issues

Therapists - available as needed

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy

Chaplains -

  • Caring for the spiritual needs of client and family

Home Health Aides -

  • Providing light housekeeping
  • Assistance with client's personal hygiene
  • Light meal preparations

Volunteers -

  • Companion to medical appointments
  • Emotional support
  • Transportation
Bereavement Support offered to family members up to one year after loss.

4 - Hospice Levels of Care

  1. Routine Home Care - is care provided to a patient in the home (or nursing home). Visits are made by the hospice staff in the patient's own environment.

  2. Continuous Care - It is an intensive, or "crisis" level of care provided to hospice patients at the routine home level of care. Continuous care is provided on a short-term basis (usually 3 to 5 days) to maintain the patient in their home or long-term care facility during an acute symptomatic crisis. Nursing care is primarily provided from 8 to 24 hours in a day to achieve palliation or management of the acute medical symptoms.

  3. Respite Care - is 5 days of care in an inpatient or nursing home setting per medicare benefit period. It is short-term care provided to the individual if necessary to relieve the family members or other persons caring for the individual to give them respite.

  4. General Inpatient Care - Short-term inpatient care is available for pain control and symptom management. This is provided for short-term care of pain or symptom control and usually is no more than 5 days. Once the symptoms are resolved and the plan of care is unchanged for 24 hours, the patient is discharged home or nursing home.
